To receive your $200 incentive, you must accumulate 50 points. Remember that you are not officially enrolled in the incentive program until you have completed and returned your Health Risk Appraisal.
Points may be accumulated by participating in:
Regular Exercise (5 points per month, August through May = 50 points total). Using the CDC definition of exercise, the participant must have at least 150 minutes of moderate-to-vigorous intensity exercise (enough to break a sweat and/or elevate the heart rate) per week each week of the month. This can be aerobic, strength training, flexibility, or any combination with an average of 30 minutes/5 days per week.
Cholesterol Screening (5 points). You must complete a new HRA in order to participate. You can have your cholesterol, bone density, and PSA levels (for men) tested during this screening. Time slots will be available from 6:00am-9:00am (dependent on availability and demand) on Thursday, September 15.
Exercise Contests (15-20 points for each contest completed).These contests encourage exercise and other healthy lifestyle habits. You must successfully complete the program to receive points. Two or more contests are offered per year, with duration of 4-8 weeks each.
Exercise Events (5-10 points each). These are campus events that promote exercise. The 5k’s will be announced later.
Wellness Seminars (5 points each). These are “Lunch and Learn” seminars that address some facet of wellness are held on campus. Times and dates will be announced later.
Wellness Consultation (10 points; only initial consultation earns points). This is a process in which you set goals for your own health/wellness, consult with the wellness coordinator to formulate a personalized plan, and follow up one month later to document your progress. Appointments are subject to availability in coordinator’s schedule and can be requested through e-mail.
Fitness Assessment (10 points, once per school year). This is a laboratory assessment of physical fitness involving tests for cardiovascular endurance (treadmill walk test), strength, flexibility, and body composition. Please allow at least 30 minutes; exercise attire is essential. This is limited to 1 appointment per person per academic year for incentive points. For additional screenings and tests, Lifesigns of Brentwood, offers EKG’s, treadmill stress tests, bone density tests, etc. which are offered at a reduced cost through insurance. Before making an appointment with Lifesigns, please contact Anna Rose Anderson for more information. Following the assessment, you would bring the report to Anna Rose Anderson to receive your points.
Personal Trainer (10 points total for 4 sessions--available SPRING semester only). You will be assigned a student personal trainer, who will assess your goals (session 1), develop a fitness plan for you (session 2), and participate in your first 2 exercise sessions with you (sessions 3 &4). Points will be awarded only if all 4 sessions are completed.
The Health Risk Appraisal (5 points for the 2011-2012 academic year) is your official entry point into the incentive program. This is a 10 minute online survey which is a tool used to evaluate your current health status. ALL employees MUST complete this for the 2011-2012 year and will be able to update it yearly.
