Your Personal Health Survey
This Personal Health Survey, an easy-to-complete questionnaire designed to help identify health risks you may face. Please complete this before your health screening. When completing this survey, remember the following:
1. This is a completely confidential survey. None of the information you provide will be shared with your employer or organization, insurance carrier or any government entity.
2. Answer every question completely and honestly. Ignoring certain questions or reporting inaccurate information will make your Personal Health Enhancement Report less valuable.
3. After completing the survey, you will receive an email directing you to your Personal Health Portal. This is where you can view your online confidential Personal Health Enhancement Report, a custom-published booklet detailing health risks you may face with suggestions on how to reduce or eliminate these risks by making simple changes to your daily routine. A few weeks after your onsite health screening you will receive an email that your Enhancement Report has been updated with your health screening results. You will also receive a hard copy of your results in the mail.
4. To complete your Personal Health Survey, use the following address and passwords to
access the survey on-line. You MUST complete this by September 15, 2011 to be included
as a member of the Faculty/Staff Wellness Program for the 2011-2012 academic year. ALL
employees desiring to be in the Faculty/Staff Wellness program MUST complete this
Personal Health Survey. You will receive 5 points for completing the Personal Health
Survey.
Log on to: ccm.personalhealthsurvey.net (do not insert “www” within the address.)
