Once you have completed 12 hours at Hazelip School of Theology, you are required to submit a Degree Progress Sheet to declare your degree intent. Students must also submit this form if they want to change their degree program. The forms are available below as both PDF and Excel documents. Open the document, type or write in your course details (professor, course number, and title, semester, and grade). Sign the sheet prior to turning it in to the HST office.
