Registration Requirements

After admission has been confirmed, graduate students are REQUIRED to submit additional documentation. Please comply with these federal and university regulations. If documents are not submitted students will NOT be able to register for courses.

Health Form: The federal government requires all students entering into a university to submit a health form for MMR and TB. Please complete the form with your physician and return it to the Health Center. (Download form here)
