Verification Information

Verification is a process to confirm the information you provided on your submitted FAFSA - Free Application for Federal Student Aid.

Verification selection can be random or because FAFSA data was incomplete, estimated or inconsistent. The U.S. Department of Education selects some students for the verification process. We will select others if we find conflicting information.

To determine if you have been selected by the Department of Education, you can review your Student Aid Report (SAR). If selected, your requirements will be listed on your myLipscomb financial aid award. You will also be notified by email if you are selected for verification by the Financial Aid Office.

We are required to ask you to provide documentation to confirm the information on your FAFSA. Documents will include a verification worksheet and may include your Federal Income Tax Return Transcript, W2 forms or copy of your Federal Income Tax Return. If you are a Dependent student, we may also request a copy of your Parent's Federal Income Tax Return Transcript, W2 forms or a copy of their Federal Income Tax Return.

The following links represent instructions and forms that are needed to complete the process.  Please print, fill out completely and sign the worksheet.

Please choose the correct worksheet for the aid year in which you are selected for verification.

• For the 2011-2012 aid year the worksheet is the same for Independent and Dependent students.  Form located to the right of this information.
•  For the 2012- 2013 aid year there are separate worksheets for Independent and Dependent students. 

When the Financial Aid Office receives all of the required documents, our staff will complete the verification process.  If a discrepancy is found, changes will be made and sent to the Department of Education.  Once the changes are processed by the Department of Education, you will receive another SAR which should reflect the changes. Once the verification is complete and any necessary changes are made, you will be awarded the appropriate financial aid.

Failure to complete the verification process may result in suspension of all federal funds and leave your student account unpaid.
