Below you will find the link to a PDF version of the Advising Agreement Form. This will allow you to print as many forms as you need at any given time, and eliminate the problem of having to find forms if you run out during the advising/registration period.
It is important--for your own protection--to keep records each semester of what your advisees agree to as far as their proposed schedule. If you have your own form that works, that's fine; this PDF is provided in case you don't. We recommend that you have the student sign whatever form you choose to utilize. It can be your discretion as to whether to give the student a copy of the form to take with them.
